Screening forms and health-related information
When you complete a screening form on this website, we may collect the responses that you enter into the form. These responses may include health-related or emotional wellbeing information, depending on the questionnaire completed.
The information you provide is used to calculate a screening score, generate a result summary, and display or generate information that you may save, print, download, or share with a healthcare provider.
Where an email address is requested, it will only be used for the purpose stated on the form, such as sending you a copy of your screening result. We will not sell, share, or use your email address for marketing, newsletters, or unrelated future contact unless you separately give permission.
Your anonymous screening responses may be stored for internal auditing, service monitoring, quality improvement and reporting purposes. Hope-Hear will not use this information to identify you or contact you unless you have provided contact details for that purpose, or unless required by law or where there is a serious safety concern.
Important safety note
Some screening tools may include questions about self-harm, suicidal thoughts, or safety concerns. These tools cannot provide emergency care.

Comments
